WILLIAM CHAPMAN INQUEST: FAMILY’S HEARTBREAKING CONCERNS ABOUT MENTAL HEALTH SUPPORT BEFORE FATAL TIGER INCIDENT

The 24-year-old died after entering a tiger enclosure at Yorkshire Wildlife Park. As investigations continue, his family is seeking answers about his contact with mental health services in the period leading up to the tragedy.

The death of William Chapman has left his family grieving while raising difficult questions about the support he received before the fatal incident at Yorkshire Wildlife Park in Doncaster, England.

William died on October 10, 2026, after entering an enclosure containing an Amur tiger named Altai. The incident prompted an emergency response and multiple investigations into the circumstances surrounding his death.

When the inquest opened at Doncaster Coroner’s Court on October 15, the hearing revealed that William had a history of mental illness, had been diagnosed with schizophrenia and had experienced previous episodes of psychosis.

His family has expressed concerns about his contact with mental health services before his death, bringing renewed attention to the questions that remain unanswered.

At the opening hearing, the coroner’s officer told the court that William had been receiving support from adult mental health services in Doncaster.

Despite his medical history, he was described during the hearing as highly functioning and independent, with no formal care requirements.

His family had raised a number of concerns about his contact with mental health services. The Rotherham, Doncaster and South Humber NHS Foundation Trust subsequently launched an investigation into its involvement with him.

William’s relatives have also spoken about his significant mental health difficulties over a number of years and the medication he had been prescribed.

However, the opening of an inquest is an initial stage in establishing the circumstances of a death. The concerns raised by his family have not, at this stage, established that any shortcoming in care contributed to the incident.

For those closest to William, the hope is that the investigations will provide a clearer understanding of the events leading up to that October afternoon.

According to evidence presented at the opening hearing, William visited Yorkshire Wildlife Park on October 10.

At approximately 1:30 p.m., he climbed over two fences and entered the tiger enclosure. He was approached by Altai, and the ensuing physical contact caused multiple traumatic injuries.

Emergency responders attended the scene, but William was pronounced dead at the wildlife park.

An initial post-mortem examination recorded multiple traumatic injuries as the cause of death.

The wildlife park cooperated with the authorities, while Doncaster Council began a separate health and safety investigation. South Yorkshire Police also gathered information about the circumstances surrounding the incident.

The tiger, Altai, was reported to be safe and was not to be euthanised following the incident.

A SECOND INVESTIGATION RAISES PRIVACY QUESTIONS

In a separate development reported on October 16, Yorkshire Ambulance Service NHS Trust launched an internal investigation after concerns that two members of staff may have accessed William’s medical records without a legitimate reason.

The trust said the potential breach had been identified through its auditing processes and reported to the Information Commissioner’s Office.

This matter concerns the handling of William’s confidential information after his death. It is separate from the investigation into his mental health care and does not, by itself, establish any connection to the circumstances of the incident.

The development has added another difficult issue for those seeking answers following William’s death.

HIS FAMILY IS LEFT WAITING FOR ANSWERS

The opening hearing was especially difficult for William’s relatives, who did not attend because they were finding the situation incredibly painful. The coroner’s team was expected to keep them informed about the proceedings.

His family has asked for the legal process to be respected while the circumstances of his death are examined.

For them, the questions extend beyond the events inside the enclosure. They also concern William’s life before that day, the care he received and whether the ongoing investigations might identify lessons that could help others.

A full inquest is scheduled for March 18, 2027.

Until then, important questions remain unresolved. The investigations will need to establish the relevant facts before conclusions can be drawn about the care William received or the circumstances that led to his death.

Behind every headline is a family mourning someone they loved. For William’s loved ones, the search for answers continues as they face a loss that cannot be undone.
