A DECADE OF HEALING: DEREK PFAFF’S TRANSFORMATIVE 60-HOUR FACE TRANSPLANT
In March 2014, 19-year-old Derek Pfaff survived a severe gunshot wound that destroyed major portions of his face, including his nose, jaws, mouth, and eyelids. Over the next decade, he underwent 58 reconstructive operations, but conventional surgery could not restore basic functions such as eating solid food, speaking clearly, blinking, or smiling.

To give Derek a chance at a normal life, surgeons at Mayo Clinic in Minnesota, led by Dr. Samir Mardini, prepared for a groundbreaking face transplant. The medical team spent nine months planning, utilizing 3D digital modeling, mapping facial nerves, and practicing the procedure 35 times using cadaver tissue.
In February 2024, a suitable donor was matched. A multidisciplinary team of over 80 healthcare professionals spent nearly 60 hours in surgery replacing approximately 85% of Derek’s face. Surgeons meticulously reconnected 18 facial nerve branches, blood vessels, muscles, skin, and bone structures to allow for eventual nerve regeneration.
Exactly ten years after his injury, on March 5, 2024, Derek looked at his new face for the first time. As his nerves gradually reconnected over the following months, he regained vital abilities—blinking, swallowing, smiling, and breathing naturally through his nose.

While Derek must take lifelong immunosuppressive medications to prevent organ rejection and faces ongoing post-transplant care, the operation restored essential daily functions. Today, alongside his recovery, Derek actively speaks publicly about suicide prevention, turning a decade of profound hardship into a message of hope and support for others.
