A newly reported coroner’s investigation has revealed disturbing details about what happened inside the room at the Rio Hotel & Casino, where Addilyn and her mother, 34-year-old Tawnia McGeehan, were discovered on February 15, 2026.

THE PAIR HAD BEEN REPORTED MISSING

Addilyn and her mother were from West Jordan, Utah, and had travelled to Las Vegas around the time of a cheerleading competition.

Concern grew when Addilyn failed to appear for the event and neither she nor her mother could be reached.

According to 911 records later released by the Las Vegas Metropolitan Police Department, Addilyn’s father contacted police seeking help locating the pair.

One of Addilyn’s cheerleading coaches also contacted emergency dispatchers after realizing that the young girl had not arrived at the competition.

The coach reportedly told dispatchers that he was concerned Addilyn could be “possibly in imminent danger.”

Police and hotel security eventually went to the room at the Rio.

Inside, they discovered Addilyn and McGeehan.

Both were pronounced dead.

A NOTE LEFT INSIDE THE ROOM

The case has now drawn renewed attention after details from a handwritten note found at the scene were reported publicly.

According to PEOPLE, citing a newly reported coroner’s report, the note instructed anyone approaching the room to contact emergency services before entering.

WHAT AUTHORITIES HAVE SAID

McGeehan’s death was ruled a suicide involving a gunshot wound, according to the coroner.
